Job Description:

The Family Case Manager will receive clients/families from the clinic and ascertain their eligibility to participate in Promoting Healthy Families Initiative or any other programs

Principle DUTIES AND RESPONSIBILITIES (not all inclusive):

 

·         Performs the initial assessment in conjunction with the client/family, to develop a written Family Service Plan (FSP).

·         Provide direct service to a minimum of 25-30 client/ families per month

·         Provide Health risk education

·         Family planning education

·         Parenting & child health education

·         Maintain accurate records & data collection

·         Referrals for diabetes, cancer, hypertension, alcohol & drug counseling.

·         Supervise the Family Outreach Workers.

·         Develops individual Perinatal Service Plans.

·         Coordination and linkages of services for clients.

·         Conduct intake assessment

·         Participate in Case Reviews

Bachelor's degree in social work or related field or Master's of social work required.  Must have two years of case management, social and/or health services work experience and a valid V.I. driver's license.  Please fax cover letter and resume to 713-8084 or email to info@vipistx.org

·         Ability to interact with people as a service liaison

·         Ability to collect data and link to community resources

·         Ability to construct client/family service plan

·         Ability to engage and refer clients for care and/or services

·         Ability to monitor the client/family service plan and make adjustments or additional referrals and follow-up consultations with VIPHC

·         Problem solving skills and ability to multi-task

·         Effective communication skills

·         Bilingual preferred

·         Computer skills, keyboarding required

·         Assessment skills using standardized tools and forms

·         Reliable car, valid insurance or reliable transportation
