RN Case Manager Care Coordinator ATS is seeking a professional with the knowledge, skills, abilities and experience who will add value to a business opportunity that we are actively pursuing. ATS is currently seeking a Care Coordinator. The Care Coordinator will provide contract and organic support assets to conduct medical readiness operations and actions across the full medical readiness spectrum. The VIARNG J-1 will act as the staff agent for VIARNG Medical Readiness. The contracted support provided will assist with all pending and future medical actions required to determine the medical readiness of all units. Further actions require capture, definition, tracking, and resolution of all pending medical issues for our Soldiers. These actions are critical to improve deployment readiness and provide the support our Soldiers and their families deserve for their actions and service to community, state, and nation. This is a Full Time position; 40 hours per week. Monday-Friday. The period of performance shall be for a basic period of 12 months: 01 November 2012 – 01 November 2013. Must be a resident of the Virgin Islands to apply. Place of performance: St. Thomas | Employment Requirements: · Bachelor’s or Master’s degree in related healthcare field. · Minimum of two years of utilization review/care coordination experience. · Current professional licensure in nursing or professional field of certification. · Appropriate certification in case management preferred — e.g., Commission for Case management Certification (CCMC). In order to successfully meet the performance objectives of this task order, the contract employee should possess the following knowledge/skills: · Knowledge and familiarity with DD Form 2766. · Knowledge of different Microsoft applications. Additional knowledge of computer software and hardware, including medical applications, is expected. · Experience working with persons in crisis. · Good documentation skills. · Excellent written and verbal communication skills, ability to establish rapport. · Ability to motivate others towards achieving goals. · Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, open personal qualities, clear sense of boundaries. Duties and Responsibilities: · Reports to the Deputy State Surgeon (DSS) and serves as the unit point of contact (POC) for all medical readiness issues. · Assists the commanders in reaching their goal of a full, medically ready mobilization roster. · Segregates Soldiers who are not medically ready from those who are medically ready and directs their documentation to the proper personnel in Virgin Islands National Guard units that best suit their current situation. · Ensures the 2766 form is complete and provides complete documentation to support the fact that a Soldier is deployable. · Maintains Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines. · Performs other responsibilities and tasks that meet the organizational needs of the Office of the State Surgeon.
