Job Description:
· Position: Patient Advocate · Time Commitment: Part-Time; 10-20 hours per week · Status: Independent Contractor · Work Location: Work from home, with occasional office visits · Reports to: Director of CSVI
JOB DESCRIPTION: · Primary Responsibilities: · Patient advocate: takes patient/physician calls, submits cancer patient requests for grants to Community Foundation of the VI (CFVI), Collaborates with Department of Human Services MAP, Schneider Regional physicians and Social Workers, ACS, St. Thomas Radiology to assist patients. · Manages the CFVI Cancer Patient fund excel sheet, logs in all requests, (which needs to be reconciled quarterly to match with CFVI fund disbursement sheet.) Runs reports to show spending on various grant categories: medical, travel to treatment, hospice, other; breast and prostate cancer. · Is creative and enjoys working within an environment that is mission driven, results driven and community oriented. · Supports the creation of educational programs for cancer awareness, prevention and advocacy · Secondary Responsibilities: · Development: Provides support service to the Director of CSVI with donor/fund relations. · Support the Director of CSVI with communications, organizational strategy, and all other special projects as assigned. |
