Job Description:

Family Outreach Educator needed to enroll clients, educate about health risk reduction, disseminate incentives, track client to follow up on referrals, schedule health appointments and various other activities. The FOE will also do outreach and make presentations at designated Community events, neighborhood centers, businesses and organizations.

	Skills Required
	Fluent in both English / Spanish...College or technical degree preferred, computer competency, strict confidentiality, strong people skills and ability to work independently as well as collaboratively.
