RN Health Coach
The RN Health Coach will work closely with the Client’s Health and Wellness team; Client's other healthcare partners, other CIGNA team members and client wellness committee (key stakeholders) in meeting the organization's health management goals. Major Duties Responsible for offering and performing biometric screenings for USVI employees, including blood pressure, body mass index (BMI), body composition (% body fat), finger stick lab screening for Total cholesterol, HDL, cholesterol ratio and glucose. Coach employees based on health risks and results of biometric data and refer employees to follow-up for physician clinical evaluation. Responsible for educating and assisting employees in completion of the online Health Assessment using biometric data from onsite screenings and/or PCP lab evaluation Deliver face to face and webinar health education seminars as appropriate to drive education and awareness in the employee population. Provide onsite one on one coaching to individuals to drive health improvement by identifying barriers, solutions and ongoing measurement throughout the change process.Document interactions and interventions as directed. Educate and refer customers to available health resources when appropriate. Utilize motivational interviewing and engagement strategies to support overall health and wellness of employees. Educate customers about the availability of decision support where multiple treatment options are available. Follow up with employees to ensure that program connectivity has been achieved and assess for educational and/or program referral needs. Work closely with client, physician community, external vendors and CIGNA Corporation operations teams. Additional duties may include: Work closely with Client and their partners in evaluating present health management program and assists in formulating plan for future programming. Consult with other onsite health and wellness resources to ensure employee’s global health needs are being addressed. Provide health and wellness education utilizing a multitude of medias including group presentations, support group facilitation, creative use of media (i.e. games, quizzes, etc), newsletter contribution, etc. Support the coordination and delivery of site Health Awareness events. Primary schedule – MondayFriday, 8am5pm However flexibility is necessary to accommodate USVI population needs. |
