Job Description:

RN Case Manager would assess and admit patients to agency.

Reassess patients as needed, after hospital admission.

Coordinate patient care with primary care physician.

Supervise LPN and Home Health Aides.

Perform chart audits and prepare quarterly reports.

On-call one weekend per month (additional pay).
